An R-MCA stroke often presents as a "lacunar-like" syndrome but with prominent behavioral changes. MRI diffusion-weighted imaging (DWI) will show a small, linear infarction in the anterior perforated substance extending into the caudate head.

The surgeon did not visualize the R-MCA pre-clipping. Post-op abulia was mistaken for "depression" or "sedation." r- mca
