The film takes place shortly after the events of Insidious: Chapter 2. Josh (Patrick Wilson) and Rebecca (Rose Byrne) are still reeling from the trauma they've experienced. However, their lives are about to get even more complicated. Josh's mother, Renai (Angus Macfadyen), is in a coma, and Josh is desperate to communicate with her from beyond the grave. Meanwhile, a mysterious figure from Josh's past, Quinn (Stefanie Sherk), comes back into his life, threatening to disrupt the fragile peace he's found.

The monster, "The Man Who Can’t Breathe," is a direct reflection of this theme. He represents suffocation—the inability to move on, the choking hold of the past.
